New Smoking Cessation Strategy in the UK: Distributing Free E-cigarettes to the Homeless to Reduce Smoking Rates Among the Impoverished

According to the authoritative British media BBC, the UK will establish pilot programs at 32 homeless assistance centers nationwide to distribute free e-cigarettes to homeless individuals, aiming to help impoverished groups quit smoking and reduce smoking rates.

The BBC reports on the trial of distributing e-cigarettes to the homeless

This initiative is being jointly conducted by several prestigious universities in England, Scotland, and Wales, including University College London, King's College London, the University of Edinburgh, and the University of East Anglia. During this process, 240 homeless individuals will receive e-cigarette kits valued at £25, while a comparison group of another 240 will receive other smoking cessation services provided by quit-smoking organizations.

In the UK, the homeless population has long been a major target for smoking cessation efforts. Professor Caitlin Notley from the University of East Anglia's medical school stated that the average smoking rate in the UK is 14.1%, but among the homeless population, this figure is as high as 70%.

“E-cigarettes are the most effective means of quitting smoking, and numerous studies have found that e-cigarettes are more effective for cessation than nicotine patches or gum, with far less harm to the body compared to traditional cigarettes,” Caitlin Notley said. “For those looking to quit smoking, e-cigarettes are an attractive option.”

The funding for this trial will be provided by the UK's National Institute for Health Research. The research team proposed that if the free e-cigarettes are more effective in helping these users quit smoking compared to the two cessation services, then in the future, homeless assistance centers could adopt this method on a large scale to reduce the likelihood of smoking-related diseases in this population.

This is not the first time the UK has attempted to use e-cigarettes as part of a national smoking cessation service. In 2021, related university scholars conducted a small-scale trial. At that time, the research team distributed free e-cigarettes to 48 homeless individuals at four homeless assistance centers in the UK and provided traditional cessation services to 32 others.

After four weeks of follow-up, the research team found that the homeless individuals who received e-cigarettes exhibited a higher success rate in quitting smoking. The results of this study were published in May last year in the National Institute for Health Research's research journal.

Smoking is one of the biggest public health issues in the UK. According to statistics from the UK government, approximately 78,000 people die from smoking each year, and many more suffer from related diseases. To reduce smoking rates and improve public health, the UK has set a goal to become a "smoke-free country" by 2030, with e-cigarettes being one of the important tools to achieve this goal.

E-cigarettes have also become the first choice for those in the UK who want to quit smoking. According to data from local quit-smoking services, from 2019 to 2020, those who used e-cigarettes to quit smoking in local cessation services had a success rate ranging from 59.7% to 74%.
