In addition to vaping devices, another newer type of product that has become popular in Japan is heated tobacco. One of the most well-known brands is IQOS.

Unlike vaping devices, which use an atomizer to turn nicotine-containing liquid into vapor, heated tobacco products use real tobacco. They simply heat the tobacco to a point where the user can inhale the tobacco flavor, but not to the point of combustion that creates smoke and ash. Because of this, some people believe there is no secondhand smoke issue. But what about the health impact of heated tobacco products?

IQOS was first launched at the end of 2014 in Nagoya, Japan, and Milan, Italy. By 2018, IQOS was being sold in 43 markets. More than 5 million smokers worldwide had already switched to IQOS. IQOS uses electronic heating technology to heat the tobacco in tobacco sticks to no more than 350°C, allowing the tobacco to release a nicotine-containing aerosol without producing smoke. Because there is no combustion, there is also no ash. This differs from cigarettes, which can exceed 600°C after being lit and produce smoke and ash during combustion.

Relevant health organizations have stated that heated tobacco products are made from tobacco, and the emissions they release, like conventional cigarettes, contain nicotine and other carcinogenic substances. The World Health Organization has also issued statements on these products, saying that heated tobacco products, like traditional cigarettes, are harmful to the human body.

Nicotine and other addictive substances contained in tobacco products stimulate the nervous system to release dopamine, making people feel comfortable and relaxed. Under repeated stimulation, however, the nervous system can become weakened, and dopamine secretion may decrease over time, requiring more and stronger nicotine stimulation. There is no safe dose for such substances, so people should stay away from all forms of tobacco products to avoid addiction.

According to Japanese research reports, heated tobacco products, like conventional cigarettes, not only contain nicotine, which is highly addictive, but also contain tar, NNN, NNK, formaldehyde, acetaldehyde, and other toxic and carcinogenic substances. In addition, the latest study published this year in the journal Tobacco Control pointed out that plastic components in heated tobacco products may release an acutely toxic and highly toxic substance, formaldehyde cyanohydrin, at around 90°C during normal heating. Because of how heated tobacco devices are used, they may also reduce the interval between uses, which could increase the user’s intake of nicotine and other harmful chemicals.

Foreign experts: heated tobacco products cannot reduce the risk of disease

Although some countries classify heated tobacco products as “tobacco products” and have approved them for sale, this does not mean they are safe, nor does it mean they have been determined to be reduced-harm products. In 2018, an advisory committee convened by the U.S. Food and Drug Administration (FDA) overwhelmingly voted against the claim that heated tobacco products can reduce the risk of tobacco-related disease, and also rejected the possibility that smokers would switch to heated tobacco products as a smoking cessation tool.

Therefore, whether or not you are already a smoker, young people in particular must take the health risks of these products seriously and stay away from all forms of tobacco products. Joint research from the United States, Canada, and the United Kingdom showed that among teenagers aged 16–19, 29.1% wanted to try vaping devices, 25.1% wanted to try heated tobacco products, and 19.3% wanted to try tobacco. This suggests that heated tobacco products may be even more attractive to young people than tobacco itself. There is no safe level of exposure to smoke emissions; inhaling them is harmful to health. Do not try anything that may be addictive or contains unknown ingredients.
